Don't know how you can possibly infer that. See the word "example" in line 1 above.  Nor how anyone can miss the word "Examples:" which precedes the list.

Yes, but ONLY in their respective columns.  That is why there are so many examples of correct usage.

See the EXAMPLES.  the first field is for TONE and the second one is for OFFSET (if needed) or RANGE.

Which is exactly correct. In this case the OFFSET is where it is supposed to be and the R20m which is human readable additional info only can slide to the left.

Again, one should infer the examples in the first column and the ones in the right column and they may be mixed in any way as long as they remain in the correct column.  After these columns, evreything is considered free text for human consumption.
